Safety Training: Accident Investigation Practice

The Challenge: Enable production floor employees to conduct thorough accident investigations in a low-stakes setting, focusing on the investigation process, required documentation, and root cause analysis.


Additional Constraints

  • Budget: Developing a high-quality simulation without exceeding budget limitations.

  • Leadership Transition: The first half of the project was developed under one project lead, while the second half was completed under a new project lead.


The Design: The module was an hour-long interactive simulation developed in an e-learning tool. It included:

  • Educational Content: An introduction to the what, why, and steps of accident investigation.

  • Interactive Scenarios: Employees could observe accidents, conduct mock interviews, review documentation, examine the production board and signage, complete required forms, and determine the root cause and make recommendations.

  • Assessment: An evaluation to identify any missed considerations in their assessments, enhancing their understanding of the investigation process.


The simulation provided autonomy to learners, allowing them to interact with various elements such as witness statements, documentation, and mock interview processes.


My Contributions

  • Client Relationship Management: Maintained ongoing communication with the client, ensuring continuity and coherence during the leadership transition. Advised on integrating this module into a broader context of safety training within the organization.

  • Instructional Design Leadership: Consulted on the feasibility and development from the initial concept to the final product. Designed and developed content using raw materials and past documentation to create realistic scenarios. Created the interactive interface and supported development.

  • Vendor Coordination: Hired and managed a graphic designer and developer. Mentored the developer on working with variables and advanced e-learning aspects. Sourced and coordinated audio talent for the main narrator and scenario characters.

  • Detailed QA Testing: Coordinated review cycles with the client. Conducted usability testing and incorporated feedback for refinement.

  • Upskilling Internal Designer: Trained the in-house designer on maintaining and updating the simulation, ensuring they were equipped to handle future iterations and refinements.
